I have worked in pharmaceutical chemistry, designing new drugs.

Obviously this is a challenging problem, but so was the protein folding problem. The issue with 'solving' disease is that it takes a lot of data, both measuring and processing (connecting that one protein being over or under-expressed may be 3 or 4 steps upstream of a disease). This is what AI may be perfect to solve. 

People are bad at predicting exponentials, especially when there is a 'moment' where the technology, all of a sudden, becomes incredibly useful. 

The famous NYTimes article is a great example of this:

Here is my 2 cents 

I think it will solve most genetic diseases because it is literally looking at code which it does very well. With CRISPR it will start with the simple one bad DNA part and expand to more complex genetic diseases. 

For environmental or diseases caused by bacteria or viruses I think it will be much more complex due to the amount of variables that may be near impossible to map or measure. 

BUT using Crispr it could slow or stop aging and improve the immune system which while not "curing" all diseases it could greatly improve human life and remove the cause (bad genetics) of 1000s of diseases.

Factually possible today and we know that it is has not been actualized and very likely will not be actualized. The final-mile aspect of tech is always missing these pie-to-the-sky predictions.

I'm not an expert myself, but I was told by a guy with a PhD in Biochemistry that AI by itself can't do a whole lot in terms of actually advancing medical science. However, AI combined with Quantum Computing could make a lot of breakthroughs very quickly. It was something about AI doing the task of creating the algorithms that could be used for Quantum computing. I don't remember exactly. Maybe somebody with some more experience and/or expertise can fill me in on this?

I'm becoming convinced we're going to get a huge amount of AI regulation in the near future, not to keep AI safe, but to prevent it from helping. No way pharmaceutical companies allow AI to cure diseases like that. We could have solved world hunger by now without the need of AI and we just simply haven't. There's too much money and power involved with keeping the masses down, and it'll be too easy for AI to lift those masses up. 

I dunno. We're getting close where AI should take off but I just can't imagine the world getting significantly better any time soon. Just can't picture it.

yet doctors miss(or misdiagnose) 5%-20% of the time. functionality/performance of AI, relies on correct(and unbiased) data, which is created from the collectors(ie: health industry professionals).


the wide variation in this estimate, makes me think that it sounds like we don't even know how much is being missed/misdiagnosed. we don't  even know how much we don't know, because it has never been properly measured.

I was part of a bone marrow study for tnbc nine years ago at UPenn and  I recently got another letter from them cuz they are doing more trials now for other types of breast cancer. It's called the surmont study at UPenn if anyone is interested. 

I would also ask about the signetera test it can detect recurrence eleven months before a scan catches a recurrence with a blood test where they look for your tumors DNA. My oncologist didn't offer it until I asked for it. Then she ordered it for every three months.

This terrifies me.  I want my life back!  I don't want to deal with breast cancer medication side effects, and I don't want cancer to come back either!  I can't find a therapist!  I am treated like I have the plague by some people!  Everyone who doesn't have breast cancer says why it will never happen to them. Or people say they will not take any treatments besides surgical removal if they get breast cancer!  Nodody gets it if they don't have breast cancer!  I'm not telling anyone else I have it.  This is such a nightmare, and I wish I could wake up and it not be real!

Please read the studies on "noncompliant" use. They've researched women who don't take it as prescribed, giving themselves 'little breaks'. If you think the stats on the benefits of using AI's are worth noting, wait till you read the % of reoccurance for noncompliant  use.

In our house, our mantra is:

Every day, thousands of people dedicate ate their lives to researching how to stop it from happening (vaccines), how to treat it, how to cure it. 

I went to the NBCC leadership summit in the spring (please reach out privately if you’d like to learn more) and literally hugged and cried to the doctor who originated vaccine trials at Pitt 40 years ago because she has toiled and worked for DECADES and now they’re making achievements. It seems pointless and thankless. And she was such a wonderful woman. I know I can’t take a vaccine but I wanted her to know that this research is important and we see her. She teared up. 

Science MATTERS. Research MATTERS. We are all here because of science and research. Herceptin was a department of defense funded research product. CDK4/6? Government funded. We all need science and it’s under attack.

I had a lumpectomy, radiation and chemo 14 years ago. I had 2 micromets in my lymph nodes out of 6 that were removed. I can tell you that most definitely those cells break off and hide. I had mammos every year and no one saw anything until they did. I'm glad it wasn't Stage 4 and "only" a local recurrence, but still... the last year and a half sucked. I take Anastrozole now each day. So far I really don't feel anything uncomfortable. Back in 2011, I took Tamoxifen for a few weeks and was in the worst place emotionally that it was the pill or my sanity. I picked sanity and had a good run for 14 years.

Thanks for sharing! Does the EndoPredict test also provide information if cdk4/6 inhibitor is recommended or not? I am taking AI. I am stage 3. My MO oncologist strongly believes CDK4/6 inhibitor is not needed because my tumor was ER+ low (PR- HERR2 -) I mentioned I would like to take it, but she says it will do more harm than good.

 However, my second opinion oncologist said my case is on the fence.

I am noticing many are asking about how this applies to them if they have DCIS. As far as I know, the difference is that DCIS cannot have spread because it is not yet invasive. So it is really a “pre-cancer” in terms of metastatic ability. 

Biopsies do not lead to tumor cells spreading. 

Everything I was writing about in my original post was about IDC that is estrogen positive with no evidence of spread as far as I had clear margins and nothing in my sentinel lymph node.

An article this week in the Washington post talks about a study which showed that intense exercise can greatly decrease breast cancer recurrence rates by making physiological changes to your blood which kill cancer cells. 

The research that came out in June in the New England Journal of Medicine suggests that the impact of intense exercise on all cancer can decrease recurrence as much as 37%. 

Just worth considering when AIs or Tamoxifen reduce recurrence risk by 50%. I prefer to do both but some ladies may not be able to tolerate the meds and thus need alternatives.

I absolutely agree with you and there is a lot of research happening. If my symptoms were worse I would ask my Doctor and explore alternatives. My mother did great on AI for 5 years. But she has chemo also. I did not want to have radiation which was my other option along with the lumpectomy. And they found more ALH and LCIS in the other breast. All of that is why I made my decision. We are all so different. Our decisions are our own and shouldn't be judged. I appreciate your response!!

This is good to know! Im so sorry you are dealing with a lot of side effects. Ive been on Anastrozole for 3 weeks and so far my side effects are mild. I feel like its the most important pill I take.

Cancer cells can travel through bloodstream if you had LVI. Even if lympth nodes are clear, LVI means higher risk of metastasis.

Is it uncommon to get this Endo predict test? No one ever talked to me about that and I did go to a Medical Center that also does cancer research and s doing trials. My lumpectomy was over a year ago so I doubt there is tissue left to test but I wish somebody would have ask me if I wanted one. I have been very worried about and asking oncology about my risk for recurrence and all I have to go on is my oncotype score. I am also stage 1 hormone positive. Thank you for sharing all of this, were you given this test because you were part of a trial or could anybody get this test?

I have the exact diagnosis and situation as you do. I don't have a gene but Mother and Grandmother both had lobular cancer. I had a DMX because my small area was small but invasive and that's all it took to make that decision. Five months out and doing ok. No radiation or chemo. I do take Anastrozole daily. I have joint and muscle aches. I was going to stop for 2-3 weeks because of an overseas trip. My oncologist suggested to not stop taking it. The estrogen levels will build again. Thanks for your story. It's nice to see someone else who has had the same thing. ❤️

This is just the info I needed. I was really wondering if chemo and all its ravaging effects were worth it, followed by 5 years of Anastrozole. It just sounds so aggressive , having clear margins and no lymph node involvement.
I am about a month into taking Anastrozole and I am feeling stiff in the joints and muscles. My measuring stick is if I can still play tennis, which I can. I will, however, have a discussion with my oncologist next month, to see if taking it every other day is an option…. If things seem to get worse.

I’m glad people are researching the hibernating cancer cells. I never want cancer to return and will put up with a lot of side effects to prevent it from happening. 
I read a report from some preliminary tests done in Britain. They had a sample of several hundred women who had breast cancer and achieved PCR with treatment. Of the subset that had cancer reoccurrence, 50 % of those women had Covid in the previous six months. That’s huge! 
Hopefully they will keep researching to see if there is a link or if it’s just a coincidence. I tend to think there is some kind of link. I got Covid six months before I developed cancer. I have an immune disease and I believe they will find that Covid somehow triggers hibernating cancer cells. But I will wait for the research to prove that before I got too upset about Covid exposure.

The generative AI tool was custom-built by experts from the European Molecular Biology Laboratory (EMBL), the German Cancer Research Centre and the University of Copenhagen, using algorithmic concepts similar to those used in large language models (LLMs)

If it’s like the rest of the AI out there, it will work ok for rich, white, highly educated, urban, cisgender, heterosexuals (i.e., people similar to the training sample), but for each of those categories that you don’t fit into, the accuracy will decrease. If you’re from a group that is severely underrepresented in health research (Native Americans, less than high school education, etc.) the chances of both false positives and false negatives will be higher. 

Also, since it was developed in Germany, where everyone has healthcare, the results will generalize poorly to the US, where the relationship between demographics and health will be very different.
